Phoenix, Arizona: Newly released documents show a teacher at an Arizona prison was alone in a room full of sex offenders in January before she was stabbed and sexually assaulted by a convicted rapist.
The records released last week show a convicted sex offender lingered in the teacher's classroom after other inmates had left and attacked her.
The inmate had just started serving a 30-year sentence for raping a Glendale woman in 2011.
Prisons spokesman Doug Nick says cameras are now being installed in prison classrooms statewide.
But he says no internal review was launched to determine if there were security flaws or if guards neglected their duties because there was no need.
A retired prison official criticized the security practices that put the woman alone with sex offenders.
